Learning from Deaths Policy
Kent and Medway NHS and Social Care Partnership Trust believes that concentrating attention on the factors that cause deaths will impact positively on all persons who use services, reducing complications, length of stay and readmission rates through improving pathways of care, reducing variability of care delivery, and early recognition and escalation of concerns.

12. Supporting staff

The Mortality Review Manager will produce a quarterly report on themes, trends and analysis from Structured Judgement reviews. This will feed into the Mortality report for TWPSMRG and Quality Committee.

The learning from deaths: a dashboard for NHS providers will be used for monitoring and reporting purposes

Datix will be used to maintain records of cases reviewed.
